Sample Usability Questionnaire

During the fist session prospective study participants are asked to fill out a questionnaire. The questions involve information about participants with regard to their lifestyle, educational background, and computer experience. The information they provide on this questionnaire is necessary to establish their eligibility to participate in the study.
